Participant transitions
When a provider closes a service line, leaves a region or winds down, we manage a planned, consent-led handover to suitable registered providers.
Participants choose who supports them. We make that choice easy: introductions, continuity of key workers where possible, and a clear timeline for families and support coordinators.
Our transition service is engaged and paid for by the exiting provider. Participant information is only shared with consent.
